The questionWhy does physician productivity vary so much within the same practice?
Short answerPhysician productivity often varies within the same practice because of how each physician's day is built, not how hard they work. Templates, scheduling rules, staff support and paperwork can differ widely from one provider to the next. To see the gap fairly, compare one measure such as collections or wRVUs per clinical day, adjust for part-time schedules, look at 12 months and study how the strongest days are set up.
Picture two physicians with the same training and the same schedule.
The first one's day runs smoothly. Patients are roomed and ready when she walks in. Her template spaces out the complex visits. Her notes are done before she leaves.
The second one's day fights her at every turn. Gaps in the morning, three new patients stacked at four o'clock, a medical assistant shared with two other providers, and charting that follows her home.
Same doctor. Different day. Very different results.

The whole practice moves: higher and more consistent. Illustrative, not actual figures.
It's the day, not the doctor
When a physician's numbers lag, the easy assumption is that the physician is the problem. It usually isn't. Look closely and the difference almost always comes down to a handful of things built around them:
- The template. How visits are spaced, and whether complex patients land when there's time for them.
- The schedule. Whether the day fills evenly or swings between empty and slammed.
- The support. How many staff are in the room, and whether patients are ready when the physician is.
- The paperwork. Whether documentation fits into the day or spills into the evening.
None of those are about how good a doctor is. All of them can be changed. That's the good news buried in the gap.
Why this matters to you
Your monthly report shows a total and an average per provider. That tells you how the practice did. It hides how differently each physician's days are going.
In specialty practices, provider productivity can vary several times over on the same schedule. That spread is revenue your practice could be earning with the people, hours and patients you already have.
And it's not only about money. The physician whose day fights her is the one charting at night, getting worn down, and maybe updating her résumé. Fixing her day is how you keep her.
How to see it without ranking anyone
The goal isn't a leaderboard. It's to find which days are working and which aren't, then copy what works.
- Pick one fair measure. For most practices, that's collections or work RVUs (wRVUs) per clinical day. Imaging-heavy specialties may use studies read per day.
- Level the field. Adjust for part-time and full-time schedules so nobody is compared unfairly.
- Look at a full year. Twelve months smooths out vacations and seasonal swings.
- Then study the days, not the doctors. Where results are strongest, look at how that day is built. That's your playbook for everyone else.
National physician productivity benchmarks can give useful context, but the comparison that helps most is inside your own practice: your strongest days, not someone else's average.
Ask yourselfIf you sat in on your strongest physician's day, what would you see that's missing from the others?
How Practice Now helps
We find where the days differ. We look at a year of your data, fairly adjusted, and spot where results are strong and where they lag, across providers and locations.
We fix the day with the physician, not to them. Together with each physician, we pick one to three changes to how their day is built: the template, the schedule, the support. In our experience, that can make a real difference in weeks, not years.
We lift the whole practice. Some fixes help every provider at once, like a cleaner billing process, better payer contracts and shared clinical protocols.
Your physicians already know how to practice medicine. Our job is to give every one of them a day that lets them.
