About Practice Now

Built by a team of clinicians and operators who have already done it, many times over.

Practice Now is a practice-improvement firm founded by clinicians and operators who grew a maternal-fetal medicine (MFM) practice into a large regional specialty network. Our founders, Berkeley Geddes, Dr. John Elliott, and Mark Wright, and our team of experts have built, scaled, and sold dozens of companies between them. The same playbook can work across specialties and practice types.

Our story

A clinical transformation that became a business transformation.

Our founders didn't learn practice improvement from a textbook. They learned it hands-on, building a maternal-fetal medicine practice and closing the gap between good ideas and the execution that delivers better outcomes.

  1. The starting point

    A referral business

The practice's maternal-fetal medicine specialists advised on high-risk patients, but the referring obstetricians kept those patients and made the final calls. Recommendations weren't always carried through the same way, outcomes varied from one referring practice to the next, and the business could only grow as fast as referrals did.

Growth plateaued at 300–400 patient visits a day, with a hard ceiling near 600.

  2. The test · 2016

    Going direct to patients

In one market, the team paired a maternal-fetal medicine specialist with certified nurse midwives and cared for patients directly instead of waiting for referrals.

Within 3–4 months, the pilot was delivering more than 80% of the births in its market.

  3. The shift

    Total care

The practice kept its referral division and added a women's care group that employs its own obstetricians, now about 35 of them. Physician-authored clinical protocols became the shared standard of care, and better contracts let physicians earn more for the same work.

  4. The proof

Outcomes data that brought payers to the table

The team benchmarked its outcomes against statewide data. Dr. Elliott made the clinical case and Berkeley led the contracting. The result was stronger contracts, including shared-savings agreements with Medicaid payers.

  5. The result

Better care, and a far stronger practice

Revenue growth of 30–40% a year since 2023, and about 30% fewer level-3 NICU admissions at the pilot site.

Results reported by the practice. Figures are approximate.

  6. The next chapter

    Practice Now

Berkeley, Dr. Elliott, and Mark Wright, Berkeley's business partner of 25 years, founded Practice Now to bring the same playbook to other practices, backed by a bench of clinical, technical, and financial experts.

What it taught us

Three principles we bring to every practice.

Outcomes first and always

Quality is a financial asset. Fewer complications cost less to deliver, and benchmarked outcomes can turn "we think we're better" into a case for a rate increase.

Speed to improvement

Fix what pays back fastest, with no more than a few priorities at a time. Early wins earn trust and fund additional changes.

Systems that retain progress

Change sticks when incentives are aligned and protocols are measured. We build the systems that keep results in place after we step back.

Leadership

The team.

Four leaders, and a bench of specialists behind every engagement.

Berkeley Geddes
Co-Founder & Chief Executive Officer

Berkeley Geddes

Berkeley is a serial entrepreneur who has built, scaled, and sold more than 45 companies and founded 8 non-profits, including multiple medical practices. He has helped raise capital from seed rounds through full venture series, and has taken part in sale processes led by tier 1 and tier 2 investment banks and private equity firms.

At the maternal-fetal medicine practice he co-founded, he led the business side of the move to total care: payer contracting, expansion into new markets, and the structure that let the practice employ its own physicians. His contracting approach turned outcomes data into better rates, including shared-savings agreements with Medicaid payers.

At Practice Now he sets strategy and works with owners, operators, and investors on revenue cycle, payer contracts, and practice value.

Dr. John Elliott
Co-Founder & Chief Medical Officer

Dr. John Elliott

Dr. Elliott has spent his career on both sides of the table: delivering care, and building the organizations that deliver it. He was a founder and physician leader at Phoenix Perinatal Associates, which merged with Neonatal Specialists in 2002 to form Obstetrix Medical Group of Phoenix. He then co-founded a maternal-fetal medicine practice, recruiting its physicians and carrying the financial risk himself, and grew it into one of the leading practices in its region.

He is internationally recognized for his care of high-order multiple pregnancies. He has delivered more than 700 sets of triplets, 128 sets of quadruplets, and 27 sets of quintuplets. According to the Elliott Center for Multiples, his quintuplet pregnancies have averaged about 33 weeks, compared with a national average of about 27. He has served on numerous professional boards and teaches at national and international meetings.

At Practice Now he works with physician leaders on the protocols, outcomes data, and payer strategy that turn quality of care into a measurable business asset.
